New Patients
New patients should arrive 15 minutes before their scheduled appointment to complete the initial paperwork. We have provided several of the necessary forms below in Adobe PDF format which you can download, print and fill in at your pace. You will need Adobe Acrobat reader software to view the PDF,

Appointments
Our Wheaton office is open Monday through Friday, 8:00 a.m. to 5:00 p.m. Please call 630.858.5400 to schedule an appointment at any of our three office locations. New patients should arrive 15 minutes before their scheduled appointment to complete the necessary paperwork. Patients are asked to bring any films such as X-rays, MRIs, or other test results that pertain to their medical condition.

Emergency Care
A neurosurgeon is on call at all times for emergency situations. After hours, call our office and our answering service will contact us. If it is an extremely urgent situation, call 911 or go directly to Central DuPage Hospital and the emergency staff will contact us.

Prescription Refills
Please allow a 24-hour turnaround time for prescription refill requests. Refill requests need to be called in to the office Monday through Friday, between 8 a.m. and 5 p.m. We will call in your prescription by the next business day. This gives the nurses time to review the patient's chart and discuss the request with the doctor.

Insurance and Billing
DuPage Neurosurgery is currently contracted with several insurance and Workers Compensation carriers, in addition to accepting Medicare. Our staff can help you determine if DuPage Neurosurgery has an arrangement with your health insurance carrier.

Payment is expected at the time of service unless a contract is held with your specific insurance carrier. If a contract is held, payment will be collected according to contract.

As a courtesy, we will submit claims on behalf of our patients to their insurance companies. Patients should bring their insurance information to assure that claims are filed properly.
